How to Make an Appointment

If you need diabetes education and/or nutrition counseling, follow these steps to schedule an appointment:

  • Obtain a referral and/or prescription from your physician
  • Verify with the insurance company if diabetes education or nutrition counseling (depending on your specific needs) is covered
    • We also accept self-paying patients
  • In addition to a referral, some insurance companies require prior authorization, so please verify with your health plan if an authorization is necessary
  • Call our center at 954-265-5889 to set up the appointment. Your physician’s office also can fax the referral to 954-265-2911.
  • Please include a copy of all pertinent lab work. You also may bring these test results to your first visit or have your doctor fax them to 954-265-2911.

What to Expect During Your Appointment

If this is your first time at our center, please allow two hours for your initial visit. This will give us time to conduct a complete medical history and nutrition assessment. You will also have time to ask questions.

If you have diabetes, we will ask you several questions, including:

  • How long have you had diabetes?
  • What medications are you currently taking for diabetes?
  • Do you check your blood sugar regularly?
